Your local NHS health and wellbeing service is here to support you to feel better for the long term

The YOURhealth service delivered by Humber Teaching NHS Foundation Trust across the East Riding of Yorkshire wants you to know that personal support and motivation is available to you.

Residents can self-refer into the service to receive free one-to-one support which helps you set achievable goals, overcome barriers, and develop a personal plan to make positive changes to your health and wellbeing for the long term.

The YOURhealth team is made up of qualified NHS professionals, including Social Prescribers and Health and Wellbeing Coaches, who are on hand to support, motivate and signpost you to the most relevant and appropriate help for your individual needs.

The expert team will spend up to an hour with you at every meeting, and can meet with you weekly or fortnightly, in a way that works around your life.

Some of the topics the team cover include:

  • Loneliness
  • Depression and low mood
  • Anxiety
  • Bereavement, loss and grief
  • Sleep Hygiene
  • Employment
  • Relationships
  • Substance misuse
  • and much more

The team also offer an innovative service delivered by their Health and Wellbeing Coaches, which play a role in the wider Community Mental Health transformation programme. The coaches make sure that anyone who is struggling with their mental health and who may benefit from low to moderate level interventions can have a voice, be involved in their own care, and understand what other avenues of support may be available to them.

If you’re aged 40 to 74 and living in the East Riding of Yorkshire, you may also be eligible for a free NHS Health Check delivered by the YOURhealth service. The check includes a cholesterol and blood pressure check, as well as height, weight and waist measurements and general lifestyle improvement advice and support.
